Privacy Policy

This Privacy Policy describes how Stillwell Psychiatry, PLLC (“Stillwell Psychiatry,” “we,” “us,” or “our”) collects, uses, and shares information when you visit stillwellpsychiatry.com (the “Site”) or contact us through the Site.

Important Distinction

This policy covers the website — not your clinical record.

Health information you share during care — symptoms, diagnoses, medications, treatment notes, lab results — is Protected Health Information (PHI) under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). Our handling of PHI is governed by our separate Notice of Privacy Practices, which is provided to every patient at intake.

Please do not submit PHI through the contact form on this Site. The contact form is for general, non-clinical inquiries only.

1. Who We Are

Stillwell Psychiatry is a telehealth psychiatric practice providing care to adults located in the State of Florida. The practice is led by Rebecca Perry, APRN, PMHNP-BC (NPI 1861128803). You can reach us at:

2. Information We Collect

We collect a limited amount of information through this Site, in the categories below.

a. Information you submit through the contact form

When you complete the contact form on this Site we collect your first name, last name, email address, phone number (if you choose to provide one), and the contents of your message. The form is for general inquiries; it is not a substitute for a clinical visit, and we ask that you do not include symptoms, prescriptions, history, test results, or other PHI.

b. Information collected automatically

When you visit the Site we automatically receive certain technical information from your browser, including your IP address, approximate geographic location derived from that IP address, device type, operating system, browser type, the pages you view, the date and time of your visit, and the referring URL.

c. Information collected when you book an appointment

Appointments are scheduled through Zocdoc. When you click a “Book Appointment” link, you are directed to Zocdoc’s platform, which collects the information needed to verify your insurance and schedule your visit. Insurance billing for patients seen through Headway is processed by Headway, Inc. Information you provide on those platforms is governed by their respective privacy policies, not this one.

d. Information from cookies and similar technologies

We use Vercel Analytics to understand how visitors use the Site. Vercel Analytics is a privacy-friendly, first-party analytics tool that does not use third-party cookies and does not track visitors across websites. We do not use advertising cookies, retargeting pixels, or cross-site tracking technologies. The Site may rely on small technical cookies or local storage values required for the Site to function (for example, to remember a closed dialog).

e. We do not knowingly collect information from children

Our services are intended for adults 18 years of age or older. We do not knowingly solicit or collect personal information from children under the age of 13. If you believe a child has provided us with personal information, please contact us at stillwellpsychiatry@gmail.com so we can delete it.

3. How We Use Information

We use the information described above to:

  • Respond to inquiries you submit through the contact form and follow up by email or phone, using the contact method you provided;
  • Schedule appointments and direct you to the appropriate booking partner;
  • Operate, maintain, secure, and improve the Site, including measuring traffic and identifying broken pages;
  • Detect, prevent, and respond to fraud, abuse, or unauthorized access — for example, applying rate-limiting to the contact form to prevent spam; and
  • Comply with applicable law, respond to lawful requests, and enforce our Terms of Use.

We do not sell your personal information, and we do not share it with third parties for their own advertising or marketing purposes.

5. How We Share Information

We share information only with the limited categories of recipients listed below.

  • Hosting and infrastructure. The Site is hosted on Vercel, Inc., which processes server logs and request metadata on our behalf.
  • Analytics. Vercel Analytics processes pageview and event data to produce aggregated, privacy-friendly traffic reports.
  • Email delivery. Contact-form submissions are transmitted by Twilio SendGrid, which routes the message to our practice inbox.
  • Appointment booking. Zocdoc, Inc. handles online scheduling and insurance verification when you book an appointment.
  • Insurance billing. Headway, Inc. processes insurance claims and payments for patients seen through the Headway platform.
  • Fonts. Web fonts are served by Google Fonts; Google may receive technical request metadata when fonts are loaded.
  • Legal and protective disclosures. We may disclose information if required by law, subpoena, court order, or other valid legal process, or if we reasonably believe disclosure is necessary to protect the rights, property, or safety of Stillwell Psychiatry, our patients, or the public.
  • Business transfers. If our practice is acquired, merged, or reorganized, information may be transferred as part of that transaction, subject to the receiving party honoring the commitments in this Policy or providing notice of any material change.

Where these vendors process PHI on our behalf in the course of clinical care, they do so under a Business Associate Agreement consistent with HIPAA.

6. How We Protect Information

We use industry-standard administrative, technical, and physical safeguards designed to protect information from loss, misuse, unauthorized access, disclosure, alteration, and destruction. The Site is served over HTTPS, the contact form is rate-limited to deter abuse, and access to inbound inquiries is restricted to authorized practice staff. No method of transmission over the internet or method of electronic storage is, however, completely secure, and we cannot guarantee absolute security.

7. How Long We Keep Information

We retain contact-form submissions for as long as reasonably necessary to respond to your inquiry and maintain a record of our communications, generally up to three years. Aggregated, de-identified analytics data may be retained longer. Clinical records are retained in accordance with Florida Statutes § 456.057 and applicable HIPAA requirements, governed by our Notice of Privacy Practices.

8. Your Choices and Rights

You may choose not to submit information through the contact form. If you have submitted information and would like to review, correct, or delete it, contact us at stillwellpsychiatry@gmail.com and we will respond within a reasonable time, subject to any legal or regulatory record-keeping obligations that apply to a healthcare practice.

Most browsers let you refuse or delete cookies and clear local storage; doing so will not prevent the Site from functioning, though some non-essential features may behave differently. You can opt out of automatic analytics by enabling your browser’s “Do Not Track” or Global Privacy Control signal — we honor these signals as an opt-out where applicable.

Depending on where you live, you may have additional rights under state law (for example, the Florida Digital Bill of Rights, where applicable). Where those laws apply, you may request to access, correct, delete, or obtain a copy of personal information we hold about you, and you may appeal a denial of any such request. 9. Third-Party Links

The Site contains links to third-party services we partner with — including Zocdoc, Headway, and our review platforms. We are not responsible for the privacy practices of those third parties. Please review their privacy policies before submitting information to them.

10. Visitors From Outside the United States

The Site is operated from the United States and intended for residents of Florida. If you access the Site from outside the United States, your information will be transferred to, processed, and stored in the United States, where data-protection laws may differ from those in your jurisdiction.

11. Changes to This Policy

We may update this Privacy Policy from time to time. When we do, we will revise the “Last updated” date at the top of this page. Material changes will be flagged on the Site. Continued use of the Site after changes take effect constitutes acceptance of the revised Policy.
