Insomnia treatment begins with a 60-minute psychiatric evaluation conducted through secure telehealth. We'll map out your sleep pattern, daytime functioning, substance use, mental health, and any contributing medical issues — so we treat the actual cause and not just the symptom.
For most adults, the first move is CBT-I-focused psychotherapy — practical changes to sleep timing, stimulus control, and the habits that quietly perpetuate insomnia. When medication is the right call, medication management covers options ranging from melatonin and trazodone to short-term hypnotics, prazosin for nightmares, or treating the anxiety/depression that's actually running the bedroom.
Follow-ups stay close early on as we adjust the plan, then space out as your sleep stabilizes. If something looks like sleep apnea or another medical sleep disorder, we'll partner with a sleep medicine clinic on the pieces that need their lab.

Common questions about sleep care
Do I have to start with sleep medication?
No. For most adults with insomnia, the first-line treatment is cognitive-behavioral therapy for insomnia (CBT-I) and behavioral adjustments — not medication. Where medication is appropriate, we discuss the options (and their tradeoffs) openly so you can decide what feels right.
Could my sleep problem be caused by something else?
Often, yes. Anxiety, depression, ADHD, trauma, alcohol, late-day caffeine, undiagnosed sleep apnea, and certain medications all affect sleep. Part of the first visit is sorting out whether your sleep issue is the primary problem or a downstream symptom — and treating it where it actually starts.
Do you order sleep studies?
Stillwell doesn't directly order in-lab polysomnography, but if your evaluation suggests obstructive sleep apnea or another medical sleep disorder, we'll refer you to a sleep medicine clinic and coordinate care alongside them.
